Judy Siver sent me this information on November 20, 2012, while she was gathering information on veterans for the Hackensack High School Class of 1962 - 50 year reunion:

Thomas Edward Simmons was a Petty Officer 2nd Class and was awarded the New Jersey Vietnam Service Medal on March 11, 2003 at a ceremony in the Hall of Hero’s Law and Safety Institute, 281 Campgaw Road Mahwah, New Jersey.
